The Imitation of Christ (Imitatio Christi) by Kempis. Issued by the Poppy printery (Mohnkopf), Lübeck, 1489. The woodcut of Death with scythe was used the same year in Des Dodes Dantz, in 1520 for Dodendantz and ca. 1550 for Copenhagen's Dance of Death.
The Latin quote at the bottom, "Memorare nouissima tua & in eternum non peccabis", is also used at the end of Holbein's Dance of Death. There's a larger version here: Mohnkopf.
